Member ID cards

ID cards are mailed separately from the EOC booklet and welcome letter. Each ID card lists the subscriber's name, subscriber's ID number, effective date of coverage and group number, as well as important phone numbers and copayments for office, hospital and emergency room visits.

For the Access+ HMO® plan, we send two sets of ID cards per member to the home address of all new members. They will receive their cards within two weeks after they submit a health plan enrollment form (HBD-12 form) which is available on the CalPERS website.

Exclusive Provider Organization (EPO) plan members will receive one set of ID cards for the family and will receive the cards no more than two weeks after CalPERS provides Blue Shield with their eligibility information.

Blue Shield will assign a primary care physician to all new members. For members previously enrolled, their previous physician will be assigned to them if that physician is still available within their plan.

If your employees do not receive their ID card within the above timeframes, ask them to call Shield Concierge at (800) 334-5847, 7 a.m. to 8 p.m., seven days a week. We'll send a replacement right away. They can also register online at blueshieldca.com and print a temporary ID card. Members can begin using their Blue Shield ID cards on the date their coverage is effective.
